This page brings together the policies, agreements, waivers, and consent documents required for participation in a UDA Soccer Academy program.
These documents help players and families understand the expectations, responsibilities, safety procedures, communication standards, financial obligations, and permissions connected to the UDA experience.
Players and, where applicable, parents/guardians will be asked to review and acknowledge the applicable documents through UDA Connect.

Required Agreements
1. Player Policies & Code of Conduct
This outlines the standards expected of all UDA players, including academic commitment, attendance, behavior, team culture, communication, respect, professionalism, and academy expectations.
2. Parent/Guardian Policies & Family Code of Conduct
This outlines the expectations for parents and guardians, including communication standards, support of the player experience, respect for coaches and officials, sideline conduct, social media conduct, travel and scheduling expectations, and family responsibilities connected to the UDA program.
3. Privacy & Data Sharing Policy
This explains how UDA collects, uses, stores, and shares player information for academic, athletic, welfare, safety, administrative, communication, and program-related purposes.
4. Medical, Emergency & Physio ConsentThis explains how UDA may use relevant medical, emergency, injury, allergy, welfare, and physiotherapy-related information to support player safety, care, participation, and wellbeing.
5. Transportation Waiver
This outlines expectations and responsibilities related to transportation provided, arranged, or coordinated by UDA for training, matches, events, travel, tours, academy activities, and other program-related transportation.
6. Media, Photo, Video & Social Media Policy
This explains UDA’s expectations for social media conduct and the use of photos, videos, interviews, testimonials, player stories, match footage, training footage, and other media connected to the program.
7. Financial Responsibility Policy
This explains payment responsibilities, program fees, payment plans, deposits, deadlines, refunds, kit or apparel charges, tour or event fees, and the importance of communicating financial questions or concerns promptly.
8. Participation Risk, Liability & Hold Harmless Waiver
This explains the risks connected to athletic participation, training, matches, travel, tours, transportation, fitness activities, academy events, and other UDA-related activities, as well as the responsibilities players and families accept when participating in UDA activities.

Annual Sign-Off
Log into app.udasoccer.com with your username and password to complete the official acknowledgement and signature process inside UDA Connect.
By completing the applicable UDA Connect sign-off, the player and/or parent/guardian confirms that they have reviewed the applicable documents, understand the expectations and responsibilities, and agree to or consent to the required terms where applicable.
Players will complete the UDA Annual Player Agreements & Consents sign-off.
Parents/guardians will complete the Parent/Guardian Annual Agreements & Responsibilities sign-off.
