Player safety and wellbeing are important to UDA. This consent explains how UDA may use medical, emergency, injury, welfare, and physiotherapy-related information to support players during their participation in a UDA Soccer Academy program.
This policy applies across UDA academies and programs, including training, matches, fitness activities, travel, tours, housing, events, partner university activities, partner club activities, and other UDA-related activities.

Medical Information
Players and parents/guardians, where applicable, are responsible for providing accurate and up-to-date medical information in UDA Connect.
This may include:
Players must promptly update UDA if their medical condition, injury status, medication, allergies, emergency contacts, or support needs change.
Parent/Guardian profile information may be used in place of separate Emergency Contact information where applicable.

- Allergies
- Medications
- Medical conditions
- Injuries
- Emergency contacts
- Physical limitations
- Relevant health history
- Learning support or additional support needs
- Any information that may affect safe participation, emergency response, travel, accommodation, or program support

Emergency Medical Consent
In the event of an injury, illness, accident, emergency, or welfare-related concern, UDA may take reasonable steps to support the player’s safety and care.
This may include:
Where urgent care is required and a parent/guardian or emergency contact cannot be reached immediately, UDA may act in the best interests of the player’s health and safety to the extent permitted by applicable law.

- Contacting emergency services
- Contacting parent/guardian or emergency contacts
- Sharing relevant medical, emergency, injury, or welfare information with appropriate providers
- Assisting with access to medical care
- Communicating with UDA staff, coaches, program directors, partner universities, academy partners, club partners, or approved providers as needed
- Taking reasonable action to support the player’s health, safety, and wellbeing

Players are responsible for notifying UDA staff if they are injured, ill, unable to train, unable to compete, or have a condition that may affect participation.
UDA may require a player to sit out, modify participation, seek medical clearance, or follow a return-to-play process before resuming full activity.
Players should not participate if they know or reasonably believe that doing so may put themselves or others at risk.

Physiotherapy & Treatment Support
Where physiotherapy, athletic therapy, medical support, or treatment support is available through UDA, a partner university, an academy partner, a club partner, or an approved provider, relevant information may be shared to support assessment, treatment, recovery, and safe participation.

- Relevant communication between the player, UDA, and the provider

UDA may share relevant medical, injury, emergency, welfare, or physiotherapy information with appropriate individuals where necessary to support player safety, care, participation, or program operations.

Soccer, training, travel, matches, fitness activities, tours, transportation, housing, academy events, and program participation involve inherent risks.
UDA will take reasonable steps to support player safety, but injuries, illness, accidents, and unexpected medical situations may still occur.
Players are expected to take responsibility for their own health, communicate honestly, follow safety instructions, and update UDA promptly if their health, injury status, or support needs change.

Player Acknowledgement
By signing the annual UDA Player Agreements & Consents in UDA Connect, I acknowledge that:
- I have read and understand the UDA Medical, Emergency & Physio Consent.
- I agree to provide accurate and current medical, emergency, allergy, injury, and relevant support information.
- I agree to update UDA promptly if my medical information, emergency contacts, injury status, medication, allergies, or support needs change.
- I consent to UDA using and sharing relevant medical, emergency, injury, welfare, or physiotherapy-related information to support my safety, care, participation, and wellbeing.
- I understand that participation in sport, travel, tours, transportation, housing, fitness activities, academy events, and other UDA-related activities involves risk.
- I understand that UDA may take reasonable action in an emergency to support my care, safety, and wellbeing.
Parent/Guardian Acknowledgement, Where Applicable
By signing the Parent/Guardian Annual Agreements & Responsibilities in UDA Connect, I acknowledge that:
- I have reviewed and understand the UDA Medical, Emergency & Physio Consent.
- I confirm that the player’s medical, allergy, emergency contact, and relevant support information is accurate and current to the best of my knowledge.
- I agree to notify UDA promptly if the player’s medical information, emergency contacts, injury status, medication, allergies, or support needs change.
- In the event of an injury, illness, accident, emergency, or welfare-related concern, I authorize UDA to take reasonable steps to support the player’s safety and care, including contacting emergency services, contacting listed emergency contacts, and sharing relevant information with appropriate providers where needed.
